There are two ways to become a Canto Diem Member Patient
Option #1 -- In Person
You can join in person by bringing the original copy of your doctor's letter of recommendation and your state-issued photo ID to our Offices during regular business hours. Our hours of operation are
Mon- Sat 10:30 – 7:00 Sunday 11:00 – 4:00. If you have a Medical Cannabis ID Card, you may bring that as well. Please note that medical cannabis ID cards are optional under state law. Your membership will be pending until we verify your Letter of Recommendation. You may not be able to receive services on the same day you join.
| | | Option #2 -- By Fax
You can fax your letter of recommendation and a completed online registration form in advance of your first visit. This way, you can be sure that your letter has been verified before you arrive. To register in advance, print a copy of our online Pre-Verification Form by clicking the appropriate button above. Be sure to read our Guidance Notes for New Members carefully. It contains important information about your membership.
Fax your completed registration form and your letter of recommendation to us at 323 465 4201. It usually takes 2-3 business days to complete the verification. You can check the status of your registration by calling us during regular business hours.
